Community and Care Homes

Our Community and Care Home Programmes team works broadly on projects supporting out of hospital care, including working with NHS and other community health and social care providers, local authorities and care homes. Understanding the complexities of ageing, we focus mainly, but not exclusively, on spreading innovations which improve health or social care provision for older people.

We create networks and partnerships across a wide range of stakeholders in south London, regionally and nationally.

Our small team have skills in project management, facilitation, evaluation and transformation, and work alongside our Clinical Director, Dr Carrie Chill, an experienced GP and commissioner.
